Institutional Review Board (IRB) Request

Research with human subjects conducted by or with persons related to NOBTS requires IRB approval prior to contact with the research subjects. If your research involves obtaining information from human subjects by any means, you will need to complete the fillable form below, save it to your computer, and e-mail it to your guidance committee chair along with a pdf of the prospectus, the survey instrument or other research tool, and the informed consent. 参见Paul D. Leedy and Jeanne Ellis Ormrod, Practical Research: Planning and Design第9版., p. 103 for an example of an informed consent.


After signing the form, the chair should forward it along with the prospectus and supporting documents to redocsec@volamdolong.com no later than one month before approval is needed. For ReDOC to consider a prospectus dealing with human subjects research, the PhD office must receive the prospectus and the IRB申请表格 no later than one month prior to the ReDOC meeting at which the prospectus will be discussed. If the IRB requires changes, revised documents must be submitted to the ReDOC office to obtain IRB approval.
